What Can Happen in an Untreated or Undertreated Mallet Finger Injury?

Permanent Extension Lag

Without treatment, the injured tendon heals in an elongated position, leaving the fingertip permanently bent. While some people adapt to this droop, this may cause functional limitations.

Swan Neck Deformity

Untreated mallet finger affects the biomechanics of the extensor tendon, which leads to hyperextension of the PIP (middle) joint of the finger. This can lead to problems making a fist.

Chronic Pain and Arthritis

With untreated mallet finger, there can be uneven stresses through the joint, leading to pain, stiffness, and post-traumatic arthritis. This can cause pain, weakness and functional problems in the future.

Diminished Grip Strength

The inability to fully extend the finger can lead to weakness with grip strength, and difficulties with pinch. This can lead to loss of dexterity and manual hand performance.

Increased Risk for Reinjury

If left untreated, your finger could become stiff and have a higher risk of reinjury because the weakened tendon and altered joint mechanics make the finger more vulnerable to additional trauma.

Some patients find a distal interphalangeal extensor lag or a dorsal prominence cosmetically unsightly and will actively seek treatment for aesthetic reasons, especially when the affected finger is highly visible during social or professional interactions.
